    I do not accept insurance for visits. However, depending on your plan, you may be able to use insurance for labs, imaging, and medications.

  • Additional Costs:

    Membership and visit fees do not include the cost of labs, imaging, medications, or supplements.

  • Location Availability:

    Services are available to patients located in states where Dr. Nazzaro is currently licensed (FL, NH, OH), with additional states being added.

  • Individualized Care:

    Treatment plans—including prescriptions for hormone therapy and/or weight loss medications—are determined on an individual basis and are not guaranteed.

Billing & Membership

  • Monthly Memberships:

    May be canceled at any time. Care continues through the end of the current billing cycle.

  • Annual Memberships:

    Billed upfront and non-refundable. By choosing an annual plan, you are committing to 12 months of care.
