1. INTRODUCTION

Welcome to the websites and services operated by Doctor JNazz, LLC and It’s Always

Something Farm LLC (“we,” “our,” “us”). These Terms and Conditions (“Terms”) govern your

use of our websites, online courses, telehealth and coaching services, affiliate links, e-commerce

stores, and related digital content (“Services”).

By using our site or purchasing a product, you agree to these Terms. If you do not agree, please

do not use our Services.

2. BUSINESS INFORMATION

• Doctor JNazz, LLC, registered in the State of Ohio, provides women’s health education,

telehealth, and coaching services under the Countryside Women's Health Co and We Are

Made™ brands.

• It’s Always Something Farm LLC, also registered in the State of Ohio, provides alpaca

products, handmade items, and farm-related merchandise.

3. MEDICAL DISCLAIMER

All information on this website, including blog posts, courses, and educational content, is

provided for informational and educational purposes only and is not intended as medical

advice.

Telehealth visits provided by Doctor JNazz, LLC occur only through secure, HIPAA-compliant

platforms and licensed providers. Coaching sessions, courses, and blog materials do not establish

a physician–patient relationship.

4. TELEHEALTH SERVICESTelehealth care is provided only to patients located in states where a medical license is held at

the time of consultation, by licensed medical providers. All telehealth services will be rendered

through an approved, HIPAA-compliant platform.

Hormone Replacement Therapy (HRT) Policy

Hormone Replacement Therapy (HRT) requires ongoing medical supervision to ensure safe,

effective, and evidence-based treatment. HRT may include estrogen, progesterone, testosterone,

or combination therapies. These medications carry potential risks, including but not limited to:

breast tenderness, abnormal bleeding, mood changes, acne, fluid retention, hair changes,

polycythemia, cardiovascular events, blood clots, stroke, fertility changes, and changes in breast

and endometrial cancer risk. Treatment eligibility is determined solely at the discretion of the

prescribing provider. Breast cancer screening imaging is required prior to initiating HRT and

must be available in the Electronic Health Record for the physician to view.

Patients who schedule a one-time hormone consultation may receive an initial, limited supply

of medication without refills, if medically appropriate after evaluation. One-time appointments

do not include ongoing monitoring, messaging access, follow-up care, dose adjustments, or

continued prescription renewals.

Because ongoing HRT requires regular monitoring, lab evaluation, safety assessments, and

symptom review, all patients who wish to continue HRT beyond an initial limited

prescription must maintain an active annual membership.Membership covers the follow-up

visits, secure communication, laboratory review, dose adjustments, and cancer-screening

compliance necessary for long-term hormone management. If a membership lapses or

recommended monitoring is not completed, prescriptions will not be renewed.

By initiating or continuing HRT through this practice, you acknowledge and agree to these

requirements, understand the inherent risks of hormone therapy, and consent to ongoing

monitoring as part of safe medical care.

GLP-1 Medication Use Policy

GLP-1 receptor agonist medications (including but not limited to semaglutide, tirzepatide, and

related compounds) carry specific risks and require ongoing clinical monitoring to ensure safe

and effective use. Potential risks of GLP-1 therapy may include gastrointestinal upset, nausea or

vomiting, dehydration, malnutrition, gallbladder disease, pancreatitis, hypoglycemia, muscle

loss, thyroid C-cell tumors (in animal studies), injection site reactions, and other adverse effects.

These medications are not appropriate for every patient, and approval for treatment is at the sole

discretion of the prescribing provider.Due to the clinical oversight required for safe GLP-1 use, all patients receiving GLP-1 therapy

through my practice must maintain an active annual membership. This membership

supports the necessary follow-up appointments, secure messaging, medication adjustments,

symptom monitoring, and required lab testing. If a patient’s membership lapses for any reason,

GLP-1 prescriptions will not be renewed.

By initiating or continuing GLP-1 therapy through this practice, you acknowledge and agree to

these requirements, understand the potential risks associated with GLP-1 medications, and

consent to ongoing monitoring as part of your care.

5. COACHING & COURSES

Coaching services and digital courses are non-refundable and intended for general wellness

education only. They are not a substitute for medical evaluation or treatment.

By purchasing or participating, you agree to hold harmless Doctor JNazz, LLC and its affiliates

for any outcomes related to coaching, courses, or lifestyle guidance.

6. PRODUCT SALES & AFFILIATES

We may sell or link to products from the following:

• Thorne, ShopMy, Amazon Storefront (affiliate programs).

• It’s Always Something Farm (alpaca products).

• We Are Made™ Collection (merchandise and digital resources).

As required by the FTC, we disclose that we may earn a small commission when you purchase

through affiliate links. These links do not affect the purchase price and help support our mission.

7. INTELLECTUAL PROPERTY

All text, graphics, images, logos, and brand names including We Are Made™, Clinic to

Countryside, Countryside Women's Health Co. and Doctor JNazz are the intellectual

property of Doctor JNazz, LLC and may not be copied, reproduced, or used without written

consent.8. USER CONDUCT

You agree not to misuse our websites or Services, including but not limited to:

• Uploading harmful code or spam

• Using contact forms to solicit services

• Copying or redistributing proprietary content

9. REFUNDS & RETURNS

• Coaching and Courses: Non-refundable.

• Digital Downloads: All sales final.

• Physical Products (Merch, Alpaca Goods): Returns accepted for damaged or defective

items only within 14 days of delivery.

10. LIMITATION OF LIABILITY

We are not liable for indirect, incidental, or consequential damages arising from your use of our

websites, courses, or products.

11. GOVERNING LAW

These Terms are governed by the laws of the State of Ohio, United States.
